Job summary

The post holder will be an autonomous practitioner who will take a lead role in the clinical management of patients with specialist palliative care needs, in a community setting

Main duties of the job

The post holder will be expected to take an active role in the advanced assessment, management and treatment of patients referred to the community Macmillan Service.

They will provide comprehensive specialist palliative and end of life care sensitive to the needs of the population, including advanced clinical assessments, history taking, diagnostics, treatment planning, prescribing and evaluation.

They will utilise advanced communication skills to assist patients to proactively manage and anticipate their future health needs, ensuring that advance care planning is embedded.
