Job summary

We are looking to recruit an enthusiastic and experienced Clinical Pharmacist to play a key role in supporting the delivery of high-quality prescribing and medicines optimisation services across the practice.

The successful candidate will work closely with GPs, nurses, healthcare professionals, and PCN colleagues to help deliver the requirements of the General Medical Services (GMS) Contract, Quality and Outcomes Framework (QOF), prescribing quality initiatives, and the Network Contract Directed Enhanced Service (PCN DES).

Main duties of the job

  • Undertake Structured Medication Reviews (SMRs) and medicines optimisation activities.
  • Manage patients with long-term conditions and complex polypharmacy.
  • Support the safe and effective use of medicines through evidence-based prescribing.
  • Reconcile medicines following hospital discharge and care transitions.
  • Review and manage high-risk medicines prescribing.
  • Provide specialist medication advice to clinicians, patients, and carers.
  • Support care home and frailty services where appropriate.
  • Contribute to achievement of QOF indicators and wider contractual requirements.
  • Lead and support prescribing audits, quality improvement initiatives, and medicines safety projects.
  • Work collaboratively with PCN pharmacists and the wider multidisciplinary team to deliver PCN DES priorities.
  • Support the practice in meeting national and local prescribing targets, Medicines Optimisation objectives, and Cost Improvement Programmes.
  • Assist with implementation of NHS guidance, prescribing policies, and best practice recommendations.
  • Contribute to service development and continuous improvement within the practice.

Job description

The post-holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face and telephone structured medication reviews, manage long-term conditions, ensure systems for safe prescribing, addressing public health and social care needs of patients at the practice. The post-holder will perform review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities.

This role is pivotal to improving the quality of care and operational efficiencies so requires motivation to deliver an excellent service within general practice.

Provide proactive leadership on medicines and prescribing systems to the multidisciplinary team, patients and their carers.

Understand the pharmacy role in governance and be able to implement this appropriately within the workplace

Maximise the use of technology and management systems to provide management of safety, risk and information

Focusing on prevention of illness and personalised care, and supporting patients to make informed decisions about their care and look after their own health, by connecting them with the full range of statutory and voluntary services

Deliver training and inform of prescribing updates, mentoring and guidance to other clinicians and staff on medicine issues

Support the practices in improving the health of their patients through the rational and safe use of medicines

Undertake responsibility for areas of QoF and Quality Contract and undertake clinical medication reviews to proactively manage patients with complex polypharmacy

Co-ordinate and lead meetings ensuring key performance indicators are identified and plans for achievement shared.

Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved

Provide advice for the GP management of more complex patients / polypharmacy patients

Report medicines related incidents, contribute to investigations and root cause analyses

Participate in serious incident investigations and multidisciplinary case reviews

Computer data entry/data allocation and collation, processing and recording information in accordance with practice procedures

Initiating contact with and responding to requests from patients, other team members and associated healthcare professionals and providers

Participate in the primary care network MDT liaising with key stake-holders to ensure consistency of patient care and benefit.
