Job summary
We have a vacancy for a Practice Nurse to join our established nursing team. In addition to standard Practice Nursing duties, the successful candidate will also take the lead role in caring for and supporting our diabetic patients. Experience of management of diabetes in a primary care setting is desirable but other relevant experience will be considered.
Team members work independently, supported by other clinical colleagues if needed. Informal meetings and peer support facilitate change and development. Participation in formal meetings ensures nurses have a voice in development of services and management of change.
Our Senior Nurse will oversee the induction period and remain available for ongoing mentoring and support. Induction and training are intended to meet the needs of the individual but will be a combination of internal, on-line and external learning opportunities.
We are a small GP practice serving local village communities, striving to provide a high standard of care and services. The team consists of nurses, HCAs and a phlebotomist. Each nurse has an area in which they have developed additional skills and interest. Members of the team are encouraged to extend their knowledge and skills so that we can develop the services we offer.
Main duties of the job
This job would suit an experienced Practice Nurse, or a nurse with experience in other settings who can bring relevant, transferable skills to this role. Experience of managing and supporting patients with diabetes is essential, although experience in a relevant previous role would be considered.
Our nurses offer a range of services and treatments, including dressings, immunisations, phlebotomy, injections, health promotion, chronic disease management, and cervical smears. We are looking for someone who is
- Interested in all aspects of practice nursing and willing to extend their knowledge in new areas
- Experienced in the management and support of diabetic patients
- Computer literate and confident we use EMIS Web
- Able to manage their own time and workload
- Willing to support and enable other members of the team
- Able to complete administration tasks associated with the nursing role
- Understanding of the importance of all members of a practice team and their role in delivering patient care
- Confident to form and express opinions in meetings; listening to the views of others and willing to adopt new ways of working
- Compassionate, conscientious, and willing to go the extra mile for patients

The nursing team work closely with each other and also with other clinical and non-clinical colleagues in the practice. Our aim is to support and encourage our staff to achieve their potential and to look forward to coming to work each day.
As part of a rural community we are able to develop a close relationship and knowledge of our patients, especially those with more extensive needs who may be regular visitors to the surgery.
