Job summary
The post holder will be an Advanced Clinical Pharmacist and Independent Prescriber with experience in acute and long-term care. They will have completed an MSc in Advanced Practice, or MSC Pharmacy, and ideally have completed the Primary Care Pharmacy Education Pathway with CPPE. The post holder must be able to evidence a clinical portfolio and relevant competencies in line with the HEE national framework for advanced clinical practice. Advanced Clinical Pharmacists are expected to work across the four pillars of advanced practice; clinical, leadership, research and education.
The post holder will autonomously undertake assessment and treatment of both acute and chronic, differentiated and undifferentiated, patient presentations.
The post holder will determine a clinical diagnosis, treatment plan and appropriate discharge, or refer, as indicated whilst maintaining accurate patient records and ensuring patient safety. They will be a source of clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership with a multidisciplinary team.
Main duties of the job
Clinical responsibilities
1. To independently undertake clinical assessments, diagnose, treat anddischarge, or appropriately refer, patients with a wide range of acute and chronic illnesses.
2. To ensure patients are fully involved in the design and implementation of their treatmentplans.
3. To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where possible.
4. To refer appropriately to clinical leads/supervising GP where a second opinion is required.
5. To adapt styles of communication appropriately to promote empowerment, self management and health education for patients.
6. To attend clinical case conferences, review and clinical meetings as required.
7. To be a source of clinical expertise to less experienced staff within the team.
8. To delegate duties/tasks to other members of staff where appropriate.
9. Provide specialist advice relating to minor illness and/or injury to local healthcare teams,patients, relative, and carers.
10. To identify actual or potential clinical risks and take appropriate action using, when required, the appropriate reporting structure.
11. Able to interpret and file pathology results
12. Manage patients across all age groups, including children.
13. No restrictions on prescribing.
14. Able to see pregnant women in regards non pregnancy related issues

About us
We are a University Practice, seeking to appoint an Advanced Clinical Pharmacist-Independent Prescriber to join our team.
Bradford Student Health Service is a busy GP practice in Bradford with approximately 8,500 registered patients. We are a university practice situated near to the University of Bradford. We are a member of PCN 6.
We are looking for applicants who are energetic, enthusiastic and focused on delivering a high standard of care to our patients. You should be proficient in all aspects of general practice nurse duties.
Our clinical team consists of two GP partners, locum GP's, a Practice Nurse, a Health Care Assistant, two GP Assistants, three Clinical Pharmacists and a Pharmacy Technician. The clinical team are supported by a Practice Manager, Operations Manager, Administrators and a Medical Secretary. Our reception team work as Care Navigators providing enhanced signposting and ensuring patients have access to the appropriate clinicians or services available to them.
The practice is a consistently high performer in relation to its contractual obligations including QOF and Quality Contract services.
