Job summary

The postholder is a senior GPwER providing assessment and management of patients with ear, nose and throat conditions in a community setting, working within an agreed scope of practice at the primary-secondary care interface. The postholder works with a broader scope, greater clinical autonomy and a higher threshold of complexity than a standard GPwER post, and acts as a point of clinical escalation within the service. The post exists to expand timely, high-quality ENT care closer to home, reduce avoidable secondary care referrals, and improve equity of access for the populations served.

Main duties of the job

Clinical

  • Assess, diagnose and manage patients presenting with ENT conditions within the agreed scope of practice (ENT UK competence framework).
  • Make safe, defensible decisions on investigation, treatment, onward referral and discharge, and recognise presentations that fall outside scope or require escalation.
  • Maintain accurate, contemporaneous records and communicate clearly with consultant colleagues, referring clinicians and patients.

Supervision and clinical leadership

  • Provide day-to-day clinical supervision, advice and case discussion for other practitioners in the service.
  • Review and sign off defined elements of others' caseload as agreed in the supervision framework.
  • Support recruitment, induction and ongoing competence assurance of the clinical team.

Service development and quality improvement

  • Contribute to the design, evaluation and continuous improvement of the community ENT pathway.
  • Lead or participate in audit, outcome monitoring and service evaluation.

Education and training

  • Deliver teaching and contribute to the GPwSI/GPwER training and accreditation programme.
  • Support workforce development across the wider ENT community service.

Governance

  • Participate in clinical governance activity: audit, significant event analysis, complaints response and the duty of candour.
  • Maintain personal accreditation and work strictly within the agreed scope of practice, escalating concerns about safety or capacity.
