Job summary

The post holder will deliver a high standard of patient care as an Advanced Practitioner in General Practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

The post-holder will demonstrate safe, clinical decision-making and expert care for patients within the general practice.They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Main duties of the job

Act as an expert practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.

To work as an autonomous advanced practitioner providing high quality and timely patient centre care.

Undertake telephone triage in order to provide total triage during surgery opening hours. This will involve providing triage cover for annual leave. Triage, treat and/or refer/direct patients presenting as same day urgent to the most appropriate service.

Undertake consultations with patients as an autonomous practitioner and using own clinical judgement, diagnose, treat, refer and/or discharge patients.

Recommend, instigate and explain appropriate invasive and non-invasive diagnostic tests, investigations and interpret findings and reports.

Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.

Job description

In addition to the Key Duties/Responsibilities shown earlier, the role also requires the following:

Provide clinical advice to reception staff when required.

If an independent prescriber, supply and administer medicines in accordance with Patient Group Directives, prescribing safe, effective and appropriate medication as defined by current legislative frameworks.

Ensure complete and accurate documentation for each and every patient contact.

Work as an independent practitioner and as part of multidisciplinary teams in order to ensure patients needs are met.

Provide the choice of direct access to an Advanced Practitioner, both in the Practice and over the telephone, for the general practice population.

To take an active role in our acute home visiting service as required by the needs of the practice.

Provide a first point of contact within for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination,problem-solvingand clinical decision-making, toestablisha diagnosis and management plan.

Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care.

Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery. Contribute to high level care for patients with chronic diseases including diabetes; respiratory and cardiac conditions.

Help develop and set up new patient services and participate in initiatives to improve existing patient services.

PROFESSIONAL ROLE

Adhere to the NMC Code of Professional Conduct, or other Professional bodies as appropriate, and at all times work within the scope of professional practice.

Promote evidence- based practice through the use of the latest research-based guidelines and the development of practice-based research.

Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review.

Maintain professional registration.

Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards.

Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.

Pro-actively promote the role of the Advanced Practitioner within the Practice, and externally to key stakeholders and agencies.

Work with the team to investigate adverse clinical incidents and complaints.

Adhere to Lancaster Medical Practice policies, procedures and guidelines.

Using personal security systems within the workplace according to Practice guidelines.

Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.

Making effective use of training to update knowledge and skills.

Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.

Reporting potential risk identified.

Act as a Chaperone when requested by clinician and in accordance with the Chaperone policy.

The post-holder will:

Apply Practice policies, standards and guidance.

Discuss with other members of the team how the policies, standards and guidelines will affect own work.

Participate in audit where appropriate.
