Job summary

We currently have a job opportunity at St Georges Surgery. You can join our team in the role of Practice Nurse. This position is full time, and you will be required to work Monday to Friday as well as covering extended hours on a Monday evening. However, we are flexible for the right person and will consider applications for both part-time and full-time positions.

Main duties of the job

The ideal candidate would be an enthusiastic and forward-thinking Practice Nurse who was ready to embrace change and innovation within this well-established and dedicated team. They will be highly motivated, committed to providing excellent patient care and have experience of Practice Nursing in a Primary Care setting.Roles will include chronic disease management, spirometry and interpretation, cervical screening, childhood immunisations and travel vaccinations.

Job description

Primary Responsibilities

The following are the core responsibilities of the practice nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

b. Implement and evaluate individual treatment plans for chronic disease patients such as diabetes, CHD, COPD and Asthma

c. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

d. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

e. Be the lead Practice Nurse for Infection Control

f. Provide travel medicine services

g. Implement vaccination programmes for adults and children

h. Request pathology services as necessary

i. Process pathology results as required

j. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

k. Qualified and confident to perform cervical screening

l. Maintain accurate clinical records in conjunction with extant legislation

m. Ensure read codes are used effectively

n. To lead and support staff in managing and maintaining chronic disease registers and achieving QOF targets

o. Develop, implement and embed well woman and man clinics

p. Chaperone patients where necessary

q. Ensure awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.

r. Assist GPs with minor surgery when required

s. Be able to recognise and manage anaphylasxis according to current UK guidelines

t. Be able to perform Cardio-pumonary resuscitationg according to current UK guidelines

u. Prioritise health issues and intervene appropriately

v. Support the team in dealing with clinical emergencies

w. Recognise, assess and refer patients presenting with mental health needs

x. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)

y. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

z. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

Ours is a clinical team of seven regular GPs (five of whom are partners), one Advanced Nurse Practitioner, one Practice Nurse, one Nurse Associate and one HCA. The clinicians are well-supported with a full team of administrative and management staff.

We are a forward-looking practice that promotes the development of ARRS roles and to this end, we currently host a Physicians Associate, two Clinical Pharmacists, one Pharmacy Technician and a Mental Health Practitioner.

We value learning and development for the whole practice, and we timetable several learning events and meetings through the year. We train people in various stages of their careers, including medical students, foundation year doctors and GP trainees.
