Specialist Nurse CYPiC Lead for 18-25 year oldsNHS
Job summary
This is an exciting opportunity for a Band 6 nurse to join our ambitious, friendly and supportive Children and Young People in Care Team as a specialist nurse for care leavers aged 18-25 years, offering advice and support for our care-experienced young people to assist them with meeting their health care needs effectively. This specialist nurse post will be suited to someone wanting to provide a high quality, proactive service to enhance the health and wellbeing of our young people who have left care. We need an individual who is kind, caring and has determination and a passion to support this group and improve health outcomes.
Main duties of the job
Main duties for the postholder include being required to work collaboratively as part of a multiagency team, including the local authority, to safeguard and promote the health and wellbeing of care leavers. The postholder will need good communication skills and also knowledge, skills and experience of practicing within a safeguarding framework. Attendance at the Care Leaver Hub weekly will be required to offer a drop-in service for the young people. Within the Hub setting there will be a requirement to devise and deliver health promotion sessions, therefore the skills and experience of delivering health promotion sessions to groups will be necessary. Attendance atMulti-Agency Transition Forums to discuss individual young people as they approach leaving care will be required as well as working closely with the CYPiC specialist nurses to ensure effective transition to adult services for our young people. There will also be a requirement to carry out home visits when necessary to offer individual advice and support to young people.
