Job summary

Are you a passionate and experienced nurse ready to lead a dynamic team in delivering high-quality, patient-centred care in the community? We are seeking a motivated and visionaryTeam Leader for our North District Nursing Team.

This pivotal role combines expert clinical practice with strategic leadership. You will be responsible for the holistic assessment and treatment of complex patients, delivering evidence-based care in diverse settings including patients' homes. You'll lead efforts to reduce hospital admissions and facilitate timely discharges, promoting independence and wellbeing.

As Team Leader, you'll manage and develop a skilled team, ensuring safe caseload allocation and fostering a culture of learning and support. You'll mentor students, supervise staff, and champion service development.

You'll be the face of the team building strong relationships, influencing service pathways, and contributing to audits, recruitment, and quality improvement initiatives. Your leadership will embody the 6Cs of nursing, promoting compassionate, value-based care.

A minimum of 37.5hrs per week over 5 days may be considered for the right candidate. Cross locality working and flexibility is required for this role. Applicants must have a full valid driving licence and have access to a vehicle for work.

Main duties of the job

  • Overall accountability and responsibility for the named district nursing team
