Job summary
Are you a compassionate Registered Nurse looking to make a meaningful difference to patients and families at one of the most important times in their lives? We are looking for a Community Staff Nurse to join our Hospice at Home team, supporting the delivery of specialist palliative and end-of-life care across North West Surrey.
Working as part of our Community Services Team, you will help patients receive high-quality, personalised care in their own homes and communities, enabling them to achieve their preferred place of care wherever possible. This is a rewarding opportunity to use your clinical skills, develop your palliative care experience and work alongside a supportive multidisciplinary team.
Main duties of the job
In this role, you will provide holistic assessment, care planning, symptom management, emotional support and practical care for patients with specialist palliative and end-of-life care needs. You will work closely with Clinical Nurse Specialists, GPs, District Nursing Teams and other professionals to coordinate safe, responsive and person-centred care.
You will also maintain accurate clinical records, support families and carers, contribute to bereavement support where appropriate, and act as a positive clinical role model for junior colleagues, students and new team members.
About you
We are looking for a registered nurse with post-registration experience, excellent communication skills and a commitment to delivering compassionate, dignified and patient-centred care. You will be confident working both independently and as part of a multidisciplinary team, with the ability to manage a varied caseload and respond sensitively to the needs of patients, families and carers.
Experience in palliative care, oncology, community nursing or multi-agency working would be an advantage, although we are equally interested in hearing from nurses who can demonstrate transferable skills, a willingness to learn and a passion for hospice care.
