Job summary

Are you a motivated and compassionate Registered Nurse looking to develop your career in specialist palliative care? We are seeking an Associate Clinical Nurse Specialist to join our Community Specialist Palliative Care Team, supporting the delivery of high-quality palliative and end-of-life care across North West Surrey.

This developmental role offers an exciting opportunity to work alongside experienced Clinical Nurse Specialists and a wider multidisciplinary team, providing holistic support to patients with advanced life-limiting illnesses and their families. You will develop the knowledge, skills and experience required to progress towards a Clinical Nurse Specialist role while helping patients achieve their preferred place of care wherever possible.

Main duties of the job

In this role, you will undertake holistic assessments, contribute to care planning and coordination, deliver symptom management, and provide emotional, practical and psychological support to patients and their families. Working under the supervision of experienced Clinical Nurse Specialists and Paramedics, you will support the management of a community caseload and participate in the delivery of specialist palliative care across a range of community settings.

You will work closely with GPs, Community Nursing Teams, acute hospitals, social care providers and other healthcare professionals to ensure high-quality, responsive and person-centred care. The role also involves contributing to service development, quality improvement activities, education initiatives and the support of students and colleagues

We are looking for a registered nurse with post-registration experience, excellent communication skills and a commitment to delivering compassionate, dignified and patient-centred care. You will be confident working both independently and as part of a multidisciplinary team, with the ability to manage a varied caseload and respond sensitively to the needs of patients, families and carers.

Experience in palliative care, oncology, community nursing or multi-agency working would be an advantage, although we are equally interested in hearing from nurses who can demonstrate transferable skills, a willingness to learn and a passion for hospice care.
