Job summary
We are seeking an experienced, motivated, and dedicated Band 7 Heart Failure Nurse to join our busy, innovative, and proactive Community Heart Failure Team.
This exciting role involves delivering high-quality heart failure care to patients across primary care settings, ensuring excellent clinical outcomes and an outstanding patient experience.
The successful candidate will play a key role in providing specialist heart failure services, promoting evidence-based practice, and ensuring patients receive the highest standard of care. Working collaboratively within a multidisciplinary team (MDT), you will support seamless, patient-centred care across the healthcare pathway.Applicants should have substantial cardiology experience, with specialist knowledge and expertise in heart failure management.
You will be passionate about service excellence, committed to continuous improvement, and confident in working autonomously while contributing effectively to a wider team.
This post will involve working across the London Borough of Ealing.
Main duties of the job
- Provide specialist nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers in collaboration with the multi-disciplinary team and in line with local and national targets.
- The post holder will deputise for their line manager in the absence of their line manager. This will involve dealing with complex situations and having to take appropriate action in a variety of settings.
- To be flexible in the approach to the clinical area and to provide cover for any area specified.
- Act as a professional role model providing direct and indirect clinical care, including teaching and research.
