Job summary

Are you passionate about making a real difference in people's lives? Join our Primary Care Network as a Care Co-ordinator and play a vital role in delivering personalised, proactive and preventative care to patients within our community. Working closely with GPs, healthcare professionals, carers, community services and voluntary organisations, you will help people navigate health and care services, access the support they need, and improve their overall wellbeing.

This is a rewarding and varied role where no two days are the same. You will support patients with long-term conditions, frailty, learning disabilities, cancer diagnoses and other complex needs, helping to coordinate care, reduce health inequalities and ensure patients receive joined-up, high-quality support. You will also maintain key patient registers, contribute to proactive care programmes, support carers, and work alongside multidisciplinary teams to improve outcomes for some of our most vulnerable patients.

Main duties of the job

As a PCN Care Co-ordinator, you will support patients to access the right care, at the right time, helping them navigate health, social care and community services. You will act as a key point of contact for patients with complex needs, long-term conditions, frailty, learning disabilities, cancer diagnoses and other vulnerabilities, ensuring their care is coordinated, personalised and focused on improving their wellbeing.

Key responsibilities include:

  • Developing and reviewing personalised care and support plans with patients, carers and families.
  • Coordinating appointments, referrals and follow-up actions across health and care services.
  • Providing information, advice and signposting to community, voluntary and statutory services.
  • Supporting proactive care programmes, patient identification, risk stratification and maintenance of key patient registers.
  • Supporting carers, promoting annual health checks and helping vulnerable patients overcome barriers to accessing healthcare.
  • Working closely with GPs, nurses, pharmacists, social prescribers and partner organisations to deliver joined-up care.
  • Maintaining accurate patient records, coding activity and contributing to service improvement initiatives.
