Job summary

We are seeking an enthusiastic and motivated Clinical Pharmacist (Independent Prescriber) to join our friendly multidisciplinary team on a fixed-term basis until 31 March 2027. This role offers the opportunity to provide high-quality, patient-centred clinical care through medication reviews, independent prescribing, long-term condition management and medicines optimisation. The post is funded through the Additional Roles Reimbursement Scheme (ARRS), with the possibility of extension or a permanent position subject to funding and service requirements.

Main duties of the job

  • Undertake Structured Medication Reviews (SMRs) and medicines optimisation.
  • Independently assess, diagnose and prescribe within their scope of practice.
  • Manage patients with long-term conditions including hypertension, diabetes, asthma, COPD and cardiovascular disease.
  • Optimise prescribing in line with NICE guidance and local formularies.
  • Reconcile medicines following hospital discharge.
  • Review complex polypharmacy patients.
  • Reduce medicines waste and improve prescribing quality.
  • Support prescribing safety initiatives and audits.
  • Respond to clinical medication queries from patients and clinicians.
  • Support care home medication reviews where appropriate.
  • Work collaboratively with GPs, community pharmacists, secondary care and the wider multidisciplinary team.
  • Participate in service development and quality improvement projects.

Job Purpose

To work as an autonomous Clinical Pharmacist within the multidisciplinary team, providing expert clinical pharmacy services to patients. The post holder will undertake structured medication reviews, independently assess and prescribe for patients within their scope of practice, optimise medicines use, support long-term condition management and contribute to improving patient safety and quality of care.

Key Responsibilities - Clinical

  • Undertake Structured Medication Reviews (SMRs) for patients with complex medication needs.
  • Independently assess, diagnose and prescribe within own competency.
  • Manage patients with long-term conditions including hypertension, diabetes, asthma, COPD and cardiovascular disease.
  • Optimise prescribing in accordance with NICE guidance, local formularies and best practice.
  • Review patients with polypharmacy and complex medicines regimes.
  • Undertake medicines reconciliation following hospital discharge.
  • Respond to medication queries from patients and clinicians.
  • Identify and reduce prescribing risks and medication errors.
  • Participate in care home medication reviews where appropriate.
  • Support safe prescribing and antimicrobial stewardship initiatives.

Medicines Optimisation

  • Promote evidence-based prescribing.
  • Reduce medicines waste and improve cost-effective prescribing.
  • Support repeat prescribing systems and medication monitoring.
  • Undertake prescribing audits and implement quality improvement initiatives.
  • Contribute to QOF, Enhanced Services and other contractual requirements.

Multidisciplinary Working

  • Work collaboratively with GPs, nurses, pharmacy technicians and the wider multidisciplinary team.
  • Liaise with community pharmacies, hospital teams and other healthcare providers.
  • Provide clinical advice and education to colleagues where appropriate.
  • Participate in multidisciplinary meetings and case discussions.

Governance

  • Maintain accurate, contemporaneous clinical records using EMIS Web.
  • Work within GPhC standards, NHS England guidance and practice policies.
  • Maintain confidentiality and comply with GDPR and information governance requirements.
  • Participate in clinical audit, significant event reviews and quality improvement projects.
  • Maintain CPD and professional registration.

General

  • Support service development and innovation within the practice.
  • Participate in mandatory training and appraisal.
  • Undertake any other duties appropriate to the grade of the post.

Person Specification - Essential

  • Registered Pharmacist with the General Pharmaceutical Council (GPhC).
  • Independent Prescriber qualification.
  • Eligible to work under the NHS Additional Roles Reimbursement Scheme (ARRS).
  • Excellent clinical knowledge and consultation skills.
  • Ability to work independently and prioritise workload.
  • Excellent communication and interpersonal skills.
  • Strong IT skills including electronic clinical record systems.

Desirable

  • Experience as an Independent Prescriber managing acute minor illness presentations autonomously within primary care or an equivalent clinical setting.
  • Experience working in General Practice or Primary Care.
  • Experience using EMIS Web.
  • Completion of the CPPE Primary Care Pharmacy Education Pathway.
  • Experience in long-term condition management.
  • Experience undertaking Structured Medication Reviews.
  • Quality improvement or audit experience.

Working Pattern

  • Fixed-term contract until 31 March 2027.
  • Full-time or part-time considered.
  • Hybrid working available, with a combination of on-site clinical sessions and remote working where service needs allow.
