Job summary

As a Practice Nurse/Nursing Associate, you will be involved in a wide variety of essential nursing duties, working alongside a multidisciplinary team in a forward-thinking and patient-centred environment. We are especially interested in candidates with experience in chronic disease management to support patient care.

Main duties of the job

  • Womens Health Services Including cervical screening and contraceptive advice, in line with UK standards and guidelines.
  • Childhood Immunisations Providing vaccinations and immunisation support.
  • Travel Health Advice and Vaccinations Supporting patients with essential travel health services, including vaccination administration and patient education on travel-related health.
  • Health Promotion and Disease Prevention Offering lifestyle advice, smoking cessation support, and proactive health education to improve patients' well-being.
  • Wound Care and Management Delivering assessment and management of wounds, including dressings and suture removal, ensuring best-practice care and timely healing.
  • Infection Control and Cold Chain Management Ensuring safe infection control practices and adherence to vaccination storage protocols.

Job description

The following are the core responsibilities of the Practice Nurse/Nurse Associate. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

b. To support in the development, implementation of health promotion and wellbeing programmes.

c. To manage and support patients with chronic conditions, such as asthma, COPD, and diabetes, with an ability to provide patient-centred care plans and reviews.

d. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health.

e. Provide routine nursing care to patients including cervical screening, in accordance with clinical based evidence, NICE and the NSF.

f. Provide wound care (ulcer/doppler, etc.) to patients

g. To co-ordinate the provision of travel medicine services

h. To request pathology services and process pathology results as required

i. To maintain accurate clinical records

j. To ensure SNOMED CT codes are used effectively by all members of the nursing team

k. To ensure compliance with the practice chaperone policy

l. To assist GPs with minor surgery when required

m. To prioritise health issues and intervene appropriately

n. To support the team in dealing with clinical emergencies

o. To recognise, assess and refer patients presenting with mental health needs

p. To implement vaccination programmes for adults and children

q. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

r. To support the clinical team with all safeguarding matters, in accordance with local and national policies

s. To deliver opportunistic health promotion where appropriate
