Job summary

The Smokefree Cancer Pathways Implementation Lead will lead a programme of work within cancer pathways at HHFT to standardise and embed a culture of smokefree treatment options for cancer pathway patients who smoke and to enhance stop smoking support available from Smokefree Hampshire (SFH) who are our local authority stop smoking service (LASSS).

This is an ideal role for an experienced Tobacco Dependency knowledge expert with project management and behaviour change experience in health and social care networks and with knowledge of the HHFT, HIOW and NHSE smoke free objective. The candidate will be professionally accountable for the development and implementation of delegated objectives, tasks and duties within the Trust and HCC's joint public health programme.

The dedicated role within Hampshire Hospitals NHS Foundation Trust (HHFT) is funded by Hampshire County Council Public Health Team (HCC PH). The role is initially for 12 months with a view to embedding the role and securing further funding beyond this timeframe.

Main duties of the job

The post holder will develop collaborations and relationships within the cancer pathway network at HHFT, between departments, integrate screening pathways and tertiary services such as radiology. There may be liaison with the Wessex Cancer Alliance, the HIOW ICB, other providers, primary care, clinicians, clinical teams, pharmacy, voluntary sector and patients, carers and their families. It will be key to ensure that both formal and informal partnerships/networks are established and maintained to support successful delivery of the cancer pathway programme. Close working will be required with other corporate functions and operational leadership and guidance from the HHFT Smokefree Steering Group.

Job description

Lead the strategic and operational delivery of smoking cessation initiatives within HHFT cancer services and related services to ensure effective referral pathways, integrated electronic clinical systems, and accurate patient record management.

Drive programme delivery for tobacco control in partnership with HCC Public Health, Wessex Cancer Alliance, smokefree HHFT sites work, Smokefree Hampshire, and community stakeholders.

Embed smoking cessation conversations into cancer pathways, train staff on the impact of smoking on the risk of cancer and the outcomes of treatment.

Build staff and patient awareness of the local stop smoking service offer (Smokefree Hampshire) and motivate patients to make a quit attempt and through improving referral pathways.

Develop strong links with the HCC PH team and HIOW ICB on cohesive messaging around the HIOW and Smokefree 2030 generation plans.

Develop strong links with other local providers (eg: Primary Care Network) to share knowledge, resources and best practice to ensure consistent messaging across HIOW particularly those applicable to cancer pathways.

Follow and retain expert knowledge on smokefree policies, smoking cessation best practice and guidance (NICE, NCSCT), and attend any related county-wide tobacco control meetings, briefings, and events (where applicable).
