The Role:As a full-time Band 7 therapist specialising in frailty, you will work with doctors, advanced practitioners, nurses, therapists and pharmacists to support frail patients both in the hospital and in the community. Promoting a home first approach to the service.

Main duties of the job

Exciting Opportunity! Join the Frailty Team as a Specialist Therapist

Are you passionate about community/hospital dedicated to the care of frail patients, promoting a home first approach. We're seeking a dynamic therapist at Rochdale Infirmary to support residents of Heywood, Middleton, and Rochdale (HMR).

You'll play a key role in delivering the frailty service, ensuring timely, appropriate care is provided in patients' own homes.

You'll work closely with patients and carers to manage health needs, promote self-care, and develop personalized plans. Collaborating with multidisciplinary teams, you'll help shape high-quality, patient-centered services aligned with trust goals.
