Job summary

We are looking for 1 WTE PCN Care Coordinator/HCA with excellent communication and administrative skills to work with the GPs and other clinicians within the Mawbey Group Practice - North Lambeth PCN.

You will be working with GPs and other clinicians, to assist patients in the management of their care.

This role will be based on the needs of the practice and support the work of the PCN at practice level. These may vary from time to time depending on contractual needs.

In particular, the focus on performing annual reviews

  • NHS Health Checks
  • Diabetes reviews
  • Chronic Pain Physical reviews

In addition, maintaining recall systems in conjunction with the clinical lead and Practice manager.

This is not an exclusive list, and the role will develop over time and the needs of the practice and there is an understanding of the need for flexibility and willingness to adapt to the practice requirements including supporting the administration team as required.

You will be none judgmental, empathetic as the first point of contact for patients, clinicians and community carers. Identifying personalised care plans, engaging them in training and employment to improve their wellbeing.

Having excellent time management skills will be important to manage your workloads and deliver role objectives within the timescales

Main duties of the job

North Lambeth Primary Care Network are excited to advertise a new role in our network

As a Care coordinator you will play a crucial role in supporting patients in preparing for or in following-up clinical conversations they have with primary care professionals.

You will work closely with the GPs and other primary care professionals within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed.

As a HCA you will also be expected to provide first line basic care (within the parameter of their recognised qualifications and training) to support the nursing team in the management of patients clinical conditions.

This will include

  • Offering a regular Phlebotomy service
  • Assisting both the nursing team and administration team
  • Triage duties if appropriate
  • Basic care
  • Blood pressure
  • Weight
  • Height
  • Dressings

You will focus delivery of the comprehensive model to reflect local priorities, health inequalities or population health management risk stratification.

Key responsibilities

Scope and Authority

Scope of role and levels of authority are as defined within this job description. The postholder will be expected to undertake clinical tasks and duties that have been delegated to them and follow clear protocols.

Key result areas

  • Communication
  • Communicate effectively with other team members. Attend PCN clinical Co-ordinator meetings
  • Communicate effectively with patients and carers, being transparent about the limits of their competence, checking that patients concerns are addressed, and recognising their needs for alternative methods of communication.
  • Assist in raising awareness of health and well-being, and how it can be promoted.
  • Maintain the nursing notice board in the waiting room and promote good health with educational material liaising with patient services staff as required.
  • Respond to requests from patients and clinical staff for health information leaflets within their area of competency.
  • Use problem resolution skills to defuse difficult situations so that the patient feels heard and that their concerns are acted on and complaints are resolved.
  • Communicate immediately with senior clinical colleagues when unsure of own competency or how to address patients needs.

Information Management

  • Record information and activities undertaken with patients and carers in an accurate and timely fashion using manual or computer systems as appropriate.
  • Maintain confidentiality of information relating to patients, relatives, staff and the practice
  • Take the necessary precautions when transmitting information.
  • Use standardised codes and templates to record clinical activity for monitoring and audit.

Service development and quality

  • Ensure own actions are consistent with clinical governance systems and to practise in accordance with agreed standards of care.
  • Understand the clinical governance aspects of the role and alert senior staff of any clinical risk.
  • Contribute to the preparation of local guidelines, protocols and standards
  • Enable patients to access appropriate professionals in the team.
  • Working with members of the nursing team operate the clinical stock control
  • Be aware of current approaches, including research, to the management of patient care within general practice
  • Alert team leader and Practice managers to areas of practice that could change to enhance the effectiveness and efficiency of the practice and actively contributing to the development of an improvement culture.

Professional development

  • Take responsibility for own development and learning, including participating in supervision and recording own personal development.
  • To participate in annual appraisal process both at practice level and PCN level
  • Work with management on any new training requirements.
  • Understand the roles and responsibilities of other members of the practice.
  • Understand the legal issues pertinent to the role
  • Assist where appropriate in current clinical audit.
  • Maintain skills to administer first aid and resuscitation
  • Have a working knowledge of local Vulnerable Adults and child protection procedures

Health, Safety and Security

  • Identify and manage risk in relation to work activities
  • Adopt management of infection hazards including:
  • Hand washing
  • Universal hygiene procedures
  • Collection and handling of laboratory specimens
  • Segregation and disposal of waste materials including blood and body fluid spillages
  • Decontamination of clinical equipment
  • Reporting of sharps injuries
  • Assist patients and colleagues in adopting sound infection control measures
  • Practise safe management of microbiological and chemical hazards within the working environment according to the Control of Substances Hazardous to Health.
  • Calibrate clinical equipment as required reporting any faults to the team leader.

General

  • To undertake any reasonable duties that aid the smooth running of the practice including participating in the good housekeeping/domestic needs of the Practice.
