Job summary

We are recruiting for a full-time Practice Nurse / Senior Practice Nurse FTE 37.5 hours- to join our friendly practice team. This is a varied and interesting role, working with our patients, nurse colleagues, GPs, HCAs and other members of the team.

We are looking for an individual preferably with experience of working in a primary care environment, but we are also interested in hearing from nurses who are looking to move into primary care. Training and clinical supervision would be available for the right candidate.

The practice delivers medical and nursing services to a diverse population including a significant number of students. Experience or interest in chronic disease management, minor illness; sexual health; health promotion and mental health would be beneficial.

We seek an individual with an enthusiasm for learning; excellent communication and IT skills; effective time management and prioritisation skills and with the ability to work well individually and as part of a team.

The ideal candidate will be an enthusiastic, knowledgeable, and patient focused, who is passionate about delivering high quality care. You will bring strong clinical judgement, excellent communication skills, and a collaborative mindset. Main duties of the job

The Practice Nurse provides primary care nursing support, patient interventions and treatment within the Practice. As part of the general practice team they will work within their scope of practice and be responsible for providing patient-centred, culturally appropriate, and individual holistic care to patients.

This includes-

  • Follow-up of patient test results and communication with patients where appropriate.
  • Undertaking treatment options and carrying out appropriate clinical interventions and procedures.
  • Health screening, including NHS health checks, Asthma review, Diabetic review; Cervical screening.
  • On-going health education and advice, including counselling to improve health and prevent disease.
  • Use of recall and reminder systems and referral, as appropriate, to national screening programmes.
  • Family planning services, provision of contraceptive advice and sexual health services.
  • Delivery of immunisation programmes.
  • On-going care and support for people with chronic and terminal conditions.
  • Health promotion to the practices enrolled population, linking to public health programmes at a national, regional and local level and utilising such programmes to target specific populations.

Northgate Health Centre is situated in the heart of Oxford in a purpose built health centre which opened in 2022, with a list size of circa 13,500 patients. We have a dedicated multidisciplinary team putting patient care at the heart of everything we do. We are a supportive and friendly team of 5 GP Partners, 6 salaried GPs, 3 experienced paramedic specialists, a collaborative nursing team with friendly & efficient patient advisor and administration teams. Our practice is managed by our Head of Business, Administration & Finance and we have an Executive Assistant and Patient Services Lead who support with day-to-day operational needs. We are the named GPs for 6 of the colleges (University of Oxford) and two local care homes.

Youll be joining a friendly, closeknit team who celebrate successes together and support one another through challenges.
