Main duties of the job

Join our dedicated Community Nursing Team as a Band 5 Community Nurse and make a meaningful difference to people's lives every day. We have 2 positions available located at Warminster Hospital. Main Responsibilities include participating in assessment and care planning of patient care in conjunction with the Team Manager and support of specialist team members, communicating effectively with patients, carers, and all members of the multidisciplinary team, implementing and evaluating care and treatment in agreement with the patient, maintaining accurate patient records, implementing evidence-based care, and contributing to team meetings. It is important to prioritize daily work, initiate health promotion activities, and participate in clinical audit and service development. Candidates must hold a full UK driving license with access to a car for work purposes.

Job Introduction

Join our dedicated Community Nursing Team as a Band 5 Community Nurse and make a meaningful difference to people's lives every day.

We have 2 positions available located at Warminster Hospital.

Main Responsibilities

  • Participate in assessment and care planning of patient care in conjunction with the Team Manger and support of specialist team members.
  • Implements and evaluates care and treatment in agreement with the patient and as defined in the care plan escalates any deviation from the care plan or change in condition to the Team Manager /GP accordingly
  • Communicates effectively with patients, carers and all members of the multidisciplinary team
  • Prioritises daily work in line with clinical / patient need
  • Maintains accurate and legible patient records in line with professional and organisational requirements this includes electronic patient record system and any paper records
  • Implements and works to ensure evidence-based care / best practice
  • Actively participates and contributes to team meetings and daily handover
  • Initiates health promotion activities with individuals, patients and carers
  • Actively involved in the development and implementation of community nursing service developments and initiatives
  • Participates in clinical audit and the implementation of changes to ways of working / clinical practice as a result of findings, recommendations and action plans

The Ideal Candidate

You will be a NMC/1stlevel registered Nurse, you will have the ability to work bothindependently and also as part of a wider multidisciplinary team. It is expected that youshould have strongcommunication and organisation skills and a flexible approach which will ensureyou deliver positive experiences to patients at every stage of theirhealthcare journey.

You will have a full UK driving license with access to a car for work purposes with Business insurance for the vehicle used.

You will also need to beconfident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
