<p>Join our team as a <strong>Specialist Community Nurse (Band 6)</strong> and play a vital role in delivering high-quality, person-centred care within the community. This is an exciting opportunity for a motivated and compassionate registered nurse who is passionate about improving health outcomes, promoting independence, and supporting people to live well in their own homes.</p><p class="isSelectedEnd">As a <strong>Specialist Community Nurse (Band 6)</strong>, you will provide high-quality nursing care, treatment, advice and support to individuals within their own homes or outpatient settings, promoting and facilitating patient independence, wellbeing and self-management. Working within national and local frameworks, professional guidelines, protocols and policies, you will deliver person-centred care that meets the complex needs of patients and their families.</p><p>You will manage a caseload of patients, providing expert clinical assessment, care planning and evaluation, while acting as a clinical leader within the multidisciplinary team. The role includes providing leadership, guidance and senior clinical support to peers, junior registered staff and Support Workers within your own discipline, as well as offering specialist advice to other healthcare professionals to ensure the delivery of safe, effective and compassionate care.</p><p><strong>Please see the job descriptin attached for a ful
