Are you an enthusiastic and compassionate Nursing Associate/Practitioner looking for a rewarding role where you can make a real difference? Join our Urgent Community Response Team (UCRT) and help deliver high-quality, patient-centred care in the community, supporting people to remain safely in their own homes and avoiding hospital admissions.

UCRT is a 7-day service providing rapid assessment and treatment for patients experiencing a health crisis in their own homes, including nursing and residential care homes. Working closely with partners across Mid and South Essex--we provide holistic assessments within two hours of referral and deliver timely, coordinated care.

As a Nursing Associate/Practitioner, you will work both independently and as part of a skilled multidisciplinary team, assessing and treating patients in their own homes. Your role will include clinical assessments, monitoring observations, venepuncture, catheter care, supporting patients following falls, and managing acute episodes of illness. You will also carry out basic equipment assessments to support our Allied Health Professional colleagues, with full training provided.

This is an exciting opportunity for a proactive clinician who enjoys autonomous working and is passionate about delivering excellent community-based care.

UCRT operates 7 days a week between 8:00am and 10:00pm. The role includes weekend and bank holiday working on a shared rota.

Main duties of the job

To support the registered practitioners in UCRTo To carry out comprehensive assessments in patients own homes.o Implement and evaluate individual care plans, utilizing practical and theoreticalknowledge under the direction of a registered practitionero Maintain excellent communication with patients, relatives and members of theteam demonstrating a variety of communication skills in accordance to the patientsneeds. o To forward plan where possible for the necessary equipment required.

The post holder will provide support to the members of UCRT which include, Nurses,Allied Health Professionals and healthcare support workers to ensure that holisticassessments are completed whilst working collaboratively with the team to supportand facilitate the treatment of patients within their own home.. The postholder will be required to provide a high quality, efficient and effectiveservice to patients that are referred to UCRT, this means delivering acute care whichcould include simple reviews of patients on the UCRT caseload, monitoringobservations, venepuncture, catheter care/catheterisation assisting patients thathave fell with appropriate equipment and to support patients during an episode of illhealth within their own homeThe post holder will be proficient, knowledgeable and competent and expected towork with minimal supervision, adhering to organisational policies and proceduresand in line with their scope of practice competency. They will continue to develop intheir role through the acquisition of further skills, knowledge and competencies to bedetermined by the needs of the patients, service and teamThe role of Nurse Associate/ Practitioner is intended to provide fully integrated,holistic care and support to ensure people are safely managed at home.
