Job summary

The purpose of this role is to be an active member of the multidisciplinary team in Stepney and Wapping Neighbourhood Mental Health Team

The post holder will provide occupational therapy assessment and intervention to people in the Stepney and Wapping neighbourhood mental health community services.

To provide sessional brief input to community mental health service users of the Duty System.The role has particular focus on service users' recovery and social inclusion goals, via both individual and group work. There is care-co-ordination responsibilities. The success of the post-holder will be measured by service user outcomes and satisfaction as well as the degree to which OT input facilitates the wider service in supporting service users to step down from requiring secondary mental health care support.

To support service users' recovery by optimising independence, improving occupational performance and increasing social inclusion. Recovery intervention plans will support step down from secondary mental health services to community based universal services. Plans will be mindful of reducing costs to the service and partners and actively supporting move on plans through from supported housing and facilitating timely discharge from an inpatient settings where possible.

Main duties of the job

  • To provide an occupational therapy assessment and treatment service to patients with complex mental health problems in Tower hamlets community, in coordination with other members of the Multi-disciplinary and occupational therapy team.
  • To promote and offer a range of clinical and non-clinical interventions within the Neighborhood Mental Health Team
  • To contribute to the on-going development and transformation of the care system
  • To provide effective signposting to a range of health, social, voluntary sector and community-based resources and services.
  • To work with patients to:
  • support shared decision-making about self-management;
  • provide brief therapeutic interventions, where qualified to do so and where appropriate;
  • To provide Occupational Therapy input to service users accessing the Neighborhood, especially where needs are complex and where multiple factors impact on occupational performance.
  • To contribute to case formulations within the Neighborhood, incorporating experience and knowledge as a dually trained professional in both physical and mental health.
  • To participate at practice and network-based MDTs, share good practice and take part in training activities for other members of the MDT

Job description

  • To manage an OT-specific caseload as practicable in line with occupational

and functional recovery goals.

  • To coordinate and develop the role of Occupational Therapy within the service, reviewing the group programme in order to promote recovery and normalisation for the service user group.
  • To develop links with in-patient occupational therapy staff in order to facilitate the smooth transition for clients being discharged from the in patient service including the development of and co- facilitation of community based interventions e.g. groups
  • To care coordinate a generic caseload of service users

with complex needs.

  • To apply specialist OT clinical knowledge and skills for this client group and to carry out occupational/vocational assessments and one-to-one and group interventions.
  • To evaluate effectiveness of therapy interventions in conjunction with clients/carers, other professionals.
  • To make entries in the electronic RiO records pertaining to the OT assessment, intervention and progress of the client.
  • The service aims to maximise inclusion and high levels of individualised functioning for people in all aspects of their lives. Therefore, the post-holder will carry out his/her duties and responsibilities in a manner which is respectful to others and not knowingly show prejudice.
  • The post holder must recognise and mobilise the contributions of families, communities and colleagues to promote the wellbeing of the young people in the care of the service.
  • Carry a caseload of service users, coordinating their assessment and care planning.
  • Undertake and adhere to Safeguarding Adult and Children guidelines and principals.
  • Supervise the clinical work of junior colleagues and students as needed.
  • Contribute to clinical audit and research activities as required.
  • Participate in CPD and other developmental activities, keeping up to date with relevant research and national guidance.
  • Adhere to ELFT policies and procedures and meet the clinical governance requirements of the Trust.
  • Offer a culturally sensitive service to all clients, meeting Trust standards fair access, equality of opportunity and anti-discriminatory practice.
  • Promote the work of Stepney and Wapping NMHT by attending relevant forums and liaising with other services.
