Main duties of the job

Your role involves helping patients regain independence, improve confidence, and achieve personal rehabilitation goals following illness or injury. Working with Therapists, Nurses, and the multidisciplinary team, you will deliver rehabilitation programs, offer high-quality support to patients throughout their recovery, and monitor their progress. You'll work independently and as part of a team, contributing to service development and quality improvement initiatives. A full UK driving license and access to a vehicle is essential since you'll be traveling to various community locations and patient homes.

Package DescriptionYou will be joining a supportive and experienced multidisciplinary team who are passionate about helping patients maximise their recovery and independence following stroke and neurological conditions. The service provides rehabilitation across community settings, inpatient services and group programmes, creating a varied and rewarding working environment where no two days are the same. Main ResponsibilitiesAs a Rehabilitation Assistant, you will support the delivery of rehabilitation and care programmes for patients recovering from stroke and neurological conditions. Working under the guidance of Therapists and Nurses, you will deliver prescribed rehabilitation activities, monitor patient progress and provide valuable feedback to the wider team. You will work with patients in their own homes, within group rehabilitation sessions and across inpatient rehabilitation services as required. You will support patients with therapy programmes, mobility, daily living activities and, where required, personal care needs, helping them progress towards their rehabilitation goals. Key responsibilities include:

  • Delivering individual rehabilitation programmes under the guidance of qualified practitioners
  • Supporting patients recovering from stroke and neurological conditions
  • Assisting with group rehabilitation sessions and therapeutic activities
  • Monitoring patient progress and reporting any changes or concerns appropriately
  • Supporting patients with personal care needs where required
  • Working across community, home and inpatient settings to meet service needs
  • Maintaining accurate and timely patient records using electronic systems
  • Promoting dignity, independence and person-centred care at all times
  • Supporting service development and quality improvement initiatives
  • Working effectively as part of a multidisciplinary team to achieve the best outcomes for patients

A full UK driving licence and access to a vehicle is essential as this role involves travelling to patients' homes and community locations.

Person SpecificationEssential

  • Good standard of secondary education
  • Experience of working with the general public within a healthcare setting
  • Excellent communication and interpersonal skills
  • Ability to prioritise workload effectively
  • Ability to work both independently and as part of a team
  • Ability to recognise own limitations and work within scope of competence
  • Positive, flexible and adaptable approach
  • Good listening skills and a compassionate manner
  • Basic IT skills, including confidence using digital systems
  • Full UK driving licence and access to a vehicle for work purposes

Desirable

  • Qualification relevant to healthcare, therapy or rehabilitation
  • Previous experience working in a rehabilitation or community healthcare setting
  • Experience supporting people with neurological conditions
  • Understanding of stroke rehabilitation and person-centred care
