The Community Forensic Team is seeking an enthusiastic, compassionate, and motivatedBand 4 STaR Workerto support individuals with complex mental health needs and forensic histories in a community setting.

We are looking for a dedicated professional who is committed to delivering high-quality, person-centred care and promoting recovery-focused practice. The successful candidate will work closely with service users, their families, carers, and multidisciplinary colleagues to support positive outcomes, enhance independence, reduce risk, and enable individuals to remain safely within the community.

As a STaR Worker, you will demonstrate excellence in practice through your professionalism, reliability, and strong interpersonal skills. You will build therapeutic relationships with service users, provide practical and emotional support, and contribute to the planning and delivery of care that promotes recovery, wellbeing, and social inclusion.

Main duties of the job

The post holder will contribute to the delivery of high-quality, person-centred care that promotes independence, social inclusion, wellbeing, and positive recovery outcomes.

Key responsibilities will include:

Building and maintaining therapeutic relationships with service users, their families, carers, and partner agencies.

Supporting service users to achieve agreed recovery goals and maximise their independence within the community.

Assisting individuals to access education, employment, training, volunteering opportunities, and community resources.

Providing practical and emotional support to help service users develop daily living skills, social networks, and healthy lifestyles.

Working alongside clinicians to implement care plans, risk management plans, and relapse prevention strategies.

Monitoring service users' mental health and wellbeing and reporting concerns promptly to the appropriate clinician.

Supporting service users to engage with appointments, treatment programmes, physical health checks, and other interventions.

Contributing to the assessment and management of risk, ensuring safe and effective care is delivered at all times.

Maintaining accurate, timely, and professional clinical records in accordance with Trust policies and professional standards.

Participating in multidisciplinary meetings, reviews, and care planning processes as required.

Liaising effectively with internal and external agencies to promote coordinated and integrated care.

Develop and maintain therapeutic relationships with service users based on trust, respect, and professional boundaries.

Support service users to identify and achieve personal recovery goals.

Assist service users to develop independent living skills, confidence, and resilience.

Encourage and support engagement with mental health services, physical healthcare appointments, and community resources.

Monitor and report changes in mental health, behaviour, or risk factors to the appropriate clinician.

Contribute to relapse prevention planning and early identification of deteriorating mental health.

Promote healthy lifestyles and support service users to manage their physical health needs.

Support service users to access education, employment, volunteering, training, and meaningful occupation opportunities.

Assist individuals in developing social networks and reducing social isolation.

Facilitate access to community-based services, leisure activities, and support groups.

Promote community integration, inclusion, and participation.

Support service users in developing daily living skills, including budgeting, shopping, travel training, and maintaining accommodation where appropriate.

Work within established risk management frameworks and care plans.

Support the implementation of positive risk-taking approaches that promote recovery while maintaining safety.

Recognise and respond appropriately to risk indicators, safeguarding concerns, and changes in presentation.

Contribute information to risk assessments and multidisciplinary discussions.

Escalate concerns promptly in line with Trust policies and procedures.

Assist care coordinators and clinicians in implementing agreed interventions.

Support service users to attend appointments and engage fully in treatment programmes.

Participate in reviews, multidisciplinary meetings, case discussions, and care planning processes as required.

Liaise effectively with healthcare professionals, social care services, housing providers, criminal justice services, and voluntary agencies.

Establish and maintain effective working relationships with service users, carers, families, and partner agencies.

Communicate effectively with members of the multidisciplinary team to ensure continuity of care.
