Job summary

We are seeking an experienced and visionary leader to join Betsi Cadwaladr University Health Board as thePrimary Care Cluster Lead for Conwy West.

This influential role will provide strategic leadership across the Conwy West Cluster, bringing together primary care, community services, local authority partners, third sector organisations and wider stakeholders to improve population health outcomes and deliver high-quality, integrated care.

The successful candidate will lead the development, planning, delivery and evaluation of services that meet local population needs, whilst promoting innovation, collaboration and sustainable models of care. This is a key role in shaping the future of primary and community healthcare by driving service transformation, supporting the delivery of the Cluster Development Plan and ensuring services align with Welsh Government priorities, the Health Board's strategic objectives and the principles of Value Based Healthcare.

This role is suitable for an experienced health or social care professional who currently works within the Conwy West Cluster and has a strong understanding of primary and community services, has excellent partnership working experience, is a skilled communicator and passionate about improving local populations health and wellbeing outcomes.

Main duties of the job

The Primary Care Cluster Lead will:

  • Provide visible and effective leadership for the Conwy West Cluster, ensuring Cluster funded services are safe, effective, patient-centred and continuously improving.
  • Develop and implement a three-year Cluster Integrated Medium Term Plan (IMTP), focused on improving population health, service accessibility and sustainability.
  • Lead the development and delivery of cluster priorities which support implementation of the national Primary Care Model for Wales and the Health Board's strategic plans.
  • Work collaboratively with GPs, community teams, pharmacists, dentists, optometrists, local authority partners, third sector organisations and other stakeholders to deliver integrated services.
  • Drive service redesign, innovation and change projects that improve health and wellbeing outcomes, patient experience and value.
  • Ensure robust governance arrangements are in place across cluster services which include overseeing the cluster budgets and expenditure plans, ensuring resources are utilised effectively and in accordance with Health Board financial governance requirements.
  • Support the development of evidence-based care pathways that strengthen community-based care.

Job description

The Cluster Lead will act as a system leader across the Conwy West area, bringing together a diverse range of partners to develop and deliver services that improve health and wellbeing outcomes for local people. The role works work closely with the Integrated Health Community (IHC), Cluster Support Team and professional leaders to establish strategic priorities and ensure effective delivery of cluster objectives.

The post holder will be responsible for identifying opportunities for service improvement and transformation, using population health intelligence, performance information and stakeholder engagement to inform decision making. You will lead the planning, implementation and evaluation of projects that support integrated, person-centred care and contribute to long-term sustainability across primary and community services.

A key aspect of the role involves developing collaborative relationships across healthcare, local government, housing, public health, voluntary services and community organisations. You will influence and negotiate across organisational boundaries, facilitating constructive discussions and delivering change in complex environments where differing priorities and perspectives may exist.

You will also be responsible for maintaining effective governance arrangements, ensuring projects and services meet quality, safety and performance standards. This includes monitoring outcomes, overseeing project evaluation frameworks, producing performance reports and supporting continuous improvement activities.

The role requires strong leadership, excellent communication skills and the ability to manage complex service development programmes while balancing strategic priorities, financial responsibilities and stakeholder expectations.
