This will primarily involve collecting, recording, analysing and reporting of statistical data (as required by JCC, the Health Board's Commissioning Team and nationally), as well as being responsible for providing an accurate and timely account of the entire patient pathway, tracking the progress of patients from referral through to the eventual discharge destination. This post will particularly support the newly developed PDOC community outreach service.

The postholder will be expected to communicate and liaise with key personnel along the pathway, recording accurate information and acting as a single point of contact for specialist neurorehabilitation and PDOC services.
