Job summary

The Band 4 Nursing associate will work as part of the multidisciplinary diabetes admission avoidance team to support adults with diabetes who are at risk of hospital admission. The post holder will deliver high-quality, person centered care across community and outpatient settings, supporting early intervention, monitoring, education and coordinated care to help prevent unnecessary hospital attendance and admission.

***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Main duties of the job

The Diabetes admission avoidance team is a new service to identify patients at risk of recurrent hospital admission. The service will focus on rapid clinical assessment, early intervention and prevention of an emergency admission.

The post holder will work as part of a multidisciplinary team delivering safe, effective and compassionate care in line with Trust values and clinical standards. The role requires excellent communication, organisational skills and the ability to work flexibly in a dynamic environment with our most complex patient group. The post holder will contribute to patient assessment, care delivery and service improvement initiatives, ensuring high standards of care and a positive patient experience.

Job description

  • Deliver high-quality, person-centred care to patients under the direction and supervision of a Diabetes Specialist Nurse.
  • Support assessment, planning, implementation and evaluation of individualised care plans.
  • Undertake and record routine clinical observations and measurements, escalating concerns appropriately.
  • Accurately document care delivered and contribute to effective handover and communication within the multidisciplinary team.
  • Recognise and respond to changes in a patient's condition, escalating promptly to the registered nurse or appropriate professional.
  • Participate in health promotion, patient education and support for carers where appropriate.
  • Contribute to service improvement, audit and quality initiatives within the clinical area.
  • Maintain professional accountability, continuing professional development and compliance with NMC standards and Trust policies.
