If you want to be part of this exciting challenge then consider coming to work with CSH Surrey. We are looking for talented, forward- thinking Staff Nurses to join our Community Nursing services.

The successful applicant will be part of the Nursing workforce and will provide safe and effective care under the indirect supervision of a Senior District Nurse within the Community Nursing team. You will be working within the scope of your practice, providing, holistic nursing care to our patients living within a defined geographical location within the community. You will support and supervise HCAs, Nursing Associates and Pre-registration nurses. You may also be expected to participate in service improvement initiatives and audits. You will be supported by the service to develop your nursing skills with on-the-job training and, if appropriate, be able to take part in our internal Preceptorship Programme.

Main duties of the job

The role of the Community Nurse is to support the Community Nursing team to deliver individualised patient care in accordance with national and local priorities, organisational policies, procedures and guidelines. This includes the assessment, planning, implementation and evaluation of safe patient care and the ability to raise concerns in a timely manner when it is appropriate to do so.

You will be working as part of the Integrated Neighbourhood teams and will have close liaison with other Multi-Disciplinary Team (MDT) members. You will be required to actively participate and contribute to daily handovers and team meetings.

We expect you to prioritise your daily work in line with clinical and patient need and to maintain accurate and legible patient records using an electronic patient record system. We would also expect you to contribute to patient risk assessments, identifying any untoward situations and the reporting of these through our incident management system.

Job description

  • Participate in assessment and care planning of patient care in conjunction with the District Nurse
  • Implements and evaluates care and treatment in agreement with the patient and as defined in the care plan escalates any deviation from the care plan or change in condition to the District Nurse / GP accordingly
  • Communicates effectively with patients, carers and all members of the multidisciplinary team
  • Prioritises daily work in line with clinical / patient need
  • Maintains accurate and legible patient records in line with professional and organisational requirements this includes contemporary paper records and electronic patient record system
  • Implements and works to ensure evidence based care / best practice
  • Actively participates and contributes to team meetings and daily handover
  • Initiates health promotion activities with individuals, patients and carers
  • Takes responsibility for a defined caseload in the absence of the District Nurse, under the direction of the Practice Teacher / Team Leader
  • Actively involved in the development and implementation of community nursing service developments and initiatives
  • Participates in clinical audit and the implementation of changes to ways of working / clinical practice as a result of findings, recommendations and action plans
  • Participates in risk assessment and promotes the safety and well-being of patients, staff and any other persons at all times and assist in ensuring a safe work environment
  • Ensure that all complaints, untoward incidents, accidents and areas of clinical risk are identified and reported to the District Nurse in accordance with the organisation policies
  • Keeps up to date with professional requirements, health care developments, clinical evidence, National Service Frameworks for practice, National and Local policy and guidelines
  • Responsible for completing all own mandatory training within the required timeframes
  • Accesses and participates in regular clinical supervision and peer review
  • Participate in own annual appraisal and undertakes own continuing professional development in order to maintain and develop knowledge and skills
  • Access regular 1-1 meetings with Senior District Nurse
  • Undertake mentorship training, facilitate the teaching and learning experiences for all students and staff
  • Accountable for own professional actions in line with the NMC code and organisational policy and guidelines
  • Monitors and orders stock levels and equipment in line with budget and lean principles
  • Responsible for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintained
