<h2>Job Introduction</h2><p>As a Band 4 Associate Practitioner, you will join our valued Community Nursing Team based at Gravesham Community Hospital, Gravesend.This is an exciting opportunity for an experienced healthcare professional looking to take the next step in their career within community nursing. Working alongside Community Nurses and the wider multidisciplinary team, you will play an important role in delivering high-quality care to patients within their own homes and local communities.You will support patients with a range of healthcare needs, helping them to remain independent, manage long-term conditions and avoid unnecessary hospital admissions.
