Job summary

We are looking for an experienced Occupational Therapist keen to work in an integrated community team. We are looking for an enthusiastic, motivated clinician who is keen to carry out rapid response visits with the aim of preventing hospital admissions. Your role will also include specialist equipment provision, moving and handling assessments, falls prevention interventions and posture management. You will supervise junior therapists and health care support workers and work closely with other senior therapists in the teams.

The Chandlers ord Integrated Community Teams include community nursing, older peoples mental health, occupational therapy and physiotherapy. Our aim is to provide high quality, safe services which improve the health, wellbeing and independence of the people we care for. We deliver these services to our community so you will feel a real sense of achievement and that your work makes a difference to local people's lives. The teams are also part of the Better Local Care project and are piloting new ways of working within the community to enhance patient care.

We provide regular supervision, training and free car parking.

Main duties of the job

Work as part of the integrated locality health and social care team delivering programmes of rehabilitation within a community setting in close collaboration with other relevant services/agencies e.g. other community and inpatient teams, social care, primary and secondary care.

Manage a caseload with supervision as required/be responsible for a defined area of service, determining daily occupational therapy and multidisciplinary rehabilitation intervention, and able to plan and implement comprehensive programmes of care for people who require intensive support / rehabilitation, guided by more senior clinicians where required.

Participate in and maintain own clinical professional development (CPD) by keeping abreast of new developments and incorporating into practice under guidance from supervisor and providing supervision and assessment rehabilitation assistants and students on placement.
