Job summary

We are seeking an experienced and motivated Band 7 Clinical Specialist to join the Stroke Early Supported Discharge (ESD) service within the East Integrated Health Community of Betsi Cadwaladr University Health Board.

This is an exciting opportunity to play a key role in delivering high-quality, patient-centred rehabilitation for people recovering from stroke as they transition from hospital to home. The post holder will provide specialist assessment, treatment and clinical leadership within the multidisciplinary ESD team, supporting patients with complex rehabilitation needs and enabling safe, effective discharge from inpatient care.

Working across acute and community pathways, you will promote seamless rehabilitation, help reduce hospital length of stay and improve patient outcomes through evidence-based practice. The role also offers opportunities to work closely with the SSIR service, strengthening integrated rehabilitation approaches and supporting innovative, patient-focused care pathways.

As a senior clinician, you will contribute to service development, quality improvement and the supervision of staff and students. You will support high standards of clinical practice, drive evidence-based care and build strong partnerships with health and social care colleagues across the system.

This role is ideal for a proactive, innovative practitioner with a passion for stroke rehabilitation, integrated working and delivering high-quality care closer to home.

Main duties of the job

  • Deliver specialist stroke assessment and rehabilitation as part of the Early Supported Discharge (ESD) service.
  • Manage a complex caseload, supporting patients to transition safely from hospital to home.
  • Work collaboratively with multidisciplinary, acute, community and social care teams to provide seamless stroke rehabilitation.
  • Provide clinical leadership, advice and specialist expertise within the ESD and SSIR services.
  • Support patients and carers to maximise independence, recovery and self-management.
  • Contribute to service development, quality improvement, audit and implementation of evidence-based practice.
  • Supervise, mentor and support staff and students, promoting high standards of clinical care.
  • Maintain accurate clinical records and ensure practice meets professional, governance and stroke service standards.
  • Develop effective partnerships across health and social care to enhance integrated rehabilitation pathways.
  • Participate in ongoing professional development and help shape innovative community stroke rehabilitation services.
