Job summary

An opportunity has arisen for an enthusiastic and highly motivated social worker to join the innovative Barts Health NHS Trust High Risk Young Adults Service. This mobile service will comprise a Clinical psychologists, Social Workers and Youth Workers and will support clinical teams to engage and manage either socially vulnerable or clinically high risk individuals living with a long-term condition.

This exciting new post has been developed to improve care and outcomes for young people (age 16-25) living with a long-term condition across North East London. The post holder will support young adults and their families who may be struggling with a range of difficulties related to living with their medical condition- namely, diabetes, heart disease or kidney disease, as they make the transition from paediatric care to adult services.

As this is a new post, the post holder will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users, and to take a leading role in the future of medical services for young adults across North East London.

The post will involve cross site working across Barts health and North East London communities: Mile End Hospital, Whipps Cross Hospital, the Royal London Hospital, and St Barts Hospital.

Main duties of the job

Build strong working relationships with all members of the multidisciplinary healthcare team, both verbally and in writing. Be the primary point of contact for Children and Young People and their parent/carers accessing social care services, arranging meetings by phone or in person as required. Develop and sustain good working relations with stakeholders, other agencies working with children and young people including education and the voluntary sector to identify relevant non-clinical interventions. Attend staff meetings, supervision and training as required, internal to Barts Health and external as required.
