Job summary

We are looking for a caring and person-centred Healthcare Support Worker to join our Community Assessment Unit in Petersfield/Gosport.

You will be working within the multi-disciplinary Integrated Care Team that delivers community and clinic-based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners.

The job will include home visiting and lone-working without direct supervision and a driving licence and access to a vehicle is required for this role.

The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and older people with mental health problems.

This post is part time 22.5 hours per week.

At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.

Main duties of the job

Provide a supporting role to registered practitioners in the delivery of nursing care, respecting patients' privacy and dignity at all times

Assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners

Observe, monitor and record patient activity and health status as appropriate, communicating changes to the registered practitioners

Progress patients' treatment plans within guidelines set by the registered practitioners

Perform a range of clinical nursing skills as directed by the registered practitioners such as skin/pressure area care; vital signs; mouth/eye/nail care; catheter/bladder/stoma care; continence reassessment; bowel care; venepuncture and basic wound care

Monitor the wellbeing of vulnerable older people within current caseload whilst protecting those at risk

Monitor and maintain health, safety and security of self and others in own work

Communicate complex and sensitive information in a supportive and understandable form to patients, carers and other staff including imparting unwelcome news

Motivate and educate patients through effective communication and carry out patient-centred goal planning

Act in accordance with the Trust's record keeping policy

Assist in patients' clerical administration

Monitor equipment for the safety of self and others

Report clinical incidents and accidents to immediate superior as soon as possible

Attend all mandatory and essential training

Participate in regular supervision and appraisals
