We are looking for enthusiastic and compassionate Community Learning Disability Nurses to join our established multidisciplinary team in beautiful East Cornwall. Working within a supportive team, you will deliver person-centred care through skilled assessment, evidence-based interventions, monitoring and reviews, helping individuals achieve the best possible health and wellbeing outcomes. You will work closely with service users, families, carers and partner agencies, managing a varied and rewarding caseload across community settings. Main duties of the job

As part of the Community Learning Disability Team you will provide person-centred, skilled and effective assessment, intervention, monitoring and review for designated individuals who are eligible for the Learning Disability Service. You will be responsible for the maintenance of evidence-based learning disability nursing practice, contributing to and facilitating Care Processes and Pathways. You will manage and supervise the work of others, and act as a mentor and preceptor.

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
