Job summary

Join our happy GP practice team as a practice nurse. Please note, we are looking for a Fully Qualified Practice nurse, with experience within General Practice (Essential).

We are seeking a Practice Nurse for up to 20 hours per week, to be worked over two days at Barton House Medical Practice. The successful post holder will operate within their own sphere of competence and agreed acceptable limits of practice. The PN will need to prioritise and triage the needs of patients accordingly with use of knowledge and competencies as a qualified PN. Main duties of the job

The role will involve working as part of the multi-disciplinary team to deliver safe and high-quality care. The practice nurse duties will include (amongst others): Chronic disease management Cytology Basic wound care, ECGs, INR monitoring, Routine and childhood immunisations. Responsibilities

The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  • To work to the IPC policy
  • Develop implement and embed health promotion and well-being programmes.
  • Identify, manage, and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
  • Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Provide wound care (ulcer/doppler etc) to patients.
  • Processing and requesting pathology services/results as required.
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  • Implementation and evaluating individual treatment plans for chronic disease patients.
  • Maintain accurate clinical records in conjunction with extant legislation.
  • Ensure read codes are used effectively.
  • Assist in maintaining chronic disease registers.
  • Chaperone patients where necessary
  • Prioritise health issues and intervene appropriately.
  • Support the team in dealing with clinical emergencies.
  • Recognise, assess, and refer patients presenting with mental health needs to the GP.
  • Implement vaccination programmes for adults and children.
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
  • Work as part of the clinical team with all safeguarding matters in accordance with local and national policies
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
  • Deliver opportunistic health promotion where appropriate.
  • Work as part of the clinical team providing minor illness service (if in scope of practice)
  • Secondary responsibilities

  • In addition to the primary responsibilities, the practice nurse may be requested to:
  • Monitor and ensure the safe storage, rotation, and disposal of vaccines.
  • Support members of the nursing team including Health Care Assistants and Phlebotomists with assistance where necessary
  • Participate in local initiatives to enhance service delivery and patient care.
  • Support and participate in shared learning within the practice.
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA)

We are well supported with strong relationships with the community primary care team including a community rehabilitation team, community matron, district nurses. Our clinical team consists of supportive GP's, ANPs Practice Nurse and Health Care Assistants. With a strong management team who are always on hand to help and advice, as well as a friendly nursing team, administration, reception, and dispensary teams.
