Job summary
This is an exciting time to join our Public Health team as a Public Health Practitioner for NHS Health Checks, where you'll play a key role in reducing inequalities in cardiovascular disease (CVD) prevention, early identification and management by increasing the uptake and quality of NHS Health Checks across Hampshire.
Working as part of our wider CVD prevention programme, you'll support the development, delivery and continuous improvement of the NHS Health Checks Programme, supporting our ambition to reduce health inequalities and improve the health and wellbeing of local residents.
This varied and rewarding role sits at the heart of collaborative public health working. You'll work closely with a wide range of stakeholders and partners, including general practice, to deliver evidence-based interventions that improve health outcomes and contribute to the wider Public Health Strategy.
Main duties of the job
- Support the planning, commissioning and delivery of the NHS Health Checks Programme and related public health initiatives.
- Lead and contribute to projects that improve programme uptake, quality and outcomes across Hampshire, helping deliver the NHS Health Checks Action Plan.
- Analyse and use data, insights and evidence to inform targeted interventions and reduce health inequalities across Hampshire.
- Monitor, evaluate and report on programme performance, quality measures and key public health outcomes.
- Build and maintain effective partnerships with stakeholders including general practice, Public Health colleagues, health service providers, Integrated Care Boards, voluntary and community organisations.
- Contribute to the development of public health strategies, policies, action plans, needs assessments and service reviews.
- Prepare reports, briefings and presentations, and represent the Public Health team at meetings and events as required.
