Job summary
An exciting opportunity has arisen for an enthusiastic, experienced and patient-focused Nurse Practitioner (NP) or Advanced Nurse Practitioner (ANP) to join St Thomas Medical Group.
We are seeking a skilled clinician to provide high-quality care across our 3 sites, working autonomously within their scope of practice while being fully supported by a highly experienced multidisciplinary team.
This is a clinical role with a strong patient-facing focus and does not carry any formal leadership or line management responsibilities.
This post is offered on a part-time basis for 3 days a week.One of the working days will be either a Monday or Friday, with working days to be agreed.
The successful candidate will deliver acute minor illness clinics and play an integral role within our afternoon Duty Team, helping to provide same day access and responsive care for our patient population.
Salary will be offered between Band 7 and Band 8a equivalent, depending on experience, clinical capability and whether a recognised Advanced Clinical Practice qualification has been obtained.
Main duties of the job
Independently assess, diagnose and manage patients presenting with acute and minor illness.
Undertake clinical examination, clinical decision-making and management planning within their scope of practice.
Prescribe appropriately where qualified as an Independent Prescriber.
Manage undifferentiated presentations in a primary care setting.
Request, interpret and act upon investigations as required.
Deliver acute same-day appointments, both face-to-face and telephone consultations.
Work collaboratively as part of the afternoon multidisciplinary Duty Team alongside GPs, Paramedics, Pharmacists, Nursing colleagues and other healthcare professionals.
Contribute to safe, effective and timely patient care across the Group.
Identify and appropriately manage risk, escalating concerns when necessary.
Maintain accurate and contemporaneous clinical records.
Support the delivery of high-quality, evidence-based care and contribute to service development where appropriate.
Work within current national, local and professional guidelines to ensure the highest standards of patient care.
