A thorough understanding of psychotropic medications, including Clozapine and Lithium, is essential, as you will be responsible for supporting safe medication management and monitoring within community settings.

Main duties of the job

o Work collaboratively within a multidisciplinary team, contributing to shared decision-making and holistic care.

o Support service users prescribed psychotropic medications, including Clozapine and Lithium, in line with Trust policy and national safety standards.

o Undertake comprehensive risk assessments and develop collaborative management plans to promote safety and recovery.

o Deliver psychosocial interventions to promote independence, wellbeing, and recovery.

o Build and maintain therapeutic relationships with service users, carers, and families, embedding the principles of the Triangle of Care.

o Liaise effectively with partner agencies, primary care, and voluntary sector organisations to ensure coordinated, person-centred care.

o Contribute to a positive, learning culture, promoting reflection, innovation, and adherence to professional and Trust values.
