Job summary

We are seeking an exceptional Director of Integration & Delivery (Place) to join our newly clustered Integrated Care Boards, bringing together NHS Coventry & Warwickshire ICB and NHS Herefordshire & Worcestershire ICB.

This is a high-profile leadership role responsible for driving integration across health and care services within Herefordshire and Worcestershire. Working with local partners, providers and communities, you will lead the development and delivery of place-based strategies that improve population health, reduce inequalities and ensure high-quality, sustainable services.

Reporting to the Executive Director of Integration & Delivery, you will play a key role in shaping neighbourhood health, supporting the shift from hospital to community-based care, and delivering the ambitions of the NHS 10-Year Health Plan. You will provide visible leadership across the system, building strong partnerships and translating strategic priorities into measurable outcomes for local people.

NHS Coventry & Warwickshire ICB and NHS Herefordshire & Worcestershire ICB are working together as a cluster with a shared management team. Main duties of the job

As Director of Integration & Delivery (Place), you will provide strategic leadership for the planning, commissioning and delivery of integrated health and care services across Herefordshire and Worcestershire.

You will work closely with local authorities, NHS providers, primary care, voluntary sector partners and communities to deliver place-based priorities and improve outcomes for local populations. The role is responsible for ensuring services are aligned to local needs, supporting neighbourhood health models, driving transformation programmes and ensuring commissioned services deliver quality, value and measurable impact.

The post-holder will support the ICB's statutory responsibilities at place, promoting integration, reducing inequalities, improving service quality and ensuring meaningful public involvement in decision-making.

As Director of Integration & Delivery, you will join a senior leadership team committed to transforming health and care services across Herefordshire and Worcestershire, working at the forefront of neighbourhood health, integration and system transformation.

Job description

In summary the Director of Integration and Delivery (H&W) will:

  • Lead the development and implementation of integrated care models across neighbourhoods, places, and system levels.
  • Support the transformation of primary care on the ground and work closely with PCNs, INTs and Federations to embed them within Care Collaboratives.
  • Work with key system leaders to develop a strong tactical commissioning agenda.
  • Translate strategic commissioning plans into operational delivery, ensuring alignment with the ICBs population health strategy and the 10-Year Health Plan.
  • Ensure delivery plans are co-produced with providers, patients, and communities, and reflect local needs and priorities.
  • Build and maintain strong relationships with NHS providers, local authorities, VCSE organisations, and other system partners.
  • Support provider collaboratives and place-based partnerships to lead service transformation and delivery.
  • Facilitate joint planning, shared accountability, and integrated governance arrangements to create future Integrated Health Organisations (IHOs)
  • Monitor and evaluate the performance of lead commissioner contracts, ensuring quality, safety, and value for money.
  • Reduce unwarranted variation in conjunction with other place directors.

The post holder will ensure appropriate accountability to relevant Health and Wellbeing Boards and Health Overview and Scrutiny Committees for the delivery of place-based priorities.

All Place Directors will provide dynamic, visible leadership to deliver significant and sustained improvements in care and outcomes for the local population and the Place Director will hold collective responsibility for:

  • Care Collaborative/Place Partnerships Integration and Delivery
  • Primary Care transformation at place
  • Driving application of PHM for neighbourhood health in service delivery
  • Delivery of Neighbourhood Health model
  • Operationalising IHO development
