Job summary

An exciting opportunity has arisen within the Health and Justice service for the role of Reconnect Navigator based within the Black Country to support the strengthening of the female pathway within the service.

Reconnect supports people leaving prison and immigration centres. We will work with people up to 12 weeks prior to release and up to 6 months after release unless their health needs are met before.

The post will be based in the community however forging strong links with the female prisons will be vital to support the transition for those females being released into the Black Country.

It is expected that the post holder will form a strong working relationship with the female pathway leads from local Reconnect services and with the female pathway champions across Health and Justice within MPFT to develop the pathway and support the development of others within the team.

Reconnect is part of an integrated service with Liaison and Diversion and Mental Health Treatment Requirement services as part of the Health and Justice Service.

This is an 'all age' service and so you will be required to work with people from the age of 10 years upwards.

Prison vetting will be applied for as part of this post however please discuss any concerns regarding this as we also welcome applicants with lived experience.

Main duties of the job

o Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system or immigration centre, such as police custody, courts, and prison and community settings. Working to guidelines.o Work within limitations and escalate to IOHP as appropriate.o Provide an embedded service in police custody suites, courts and prisons working within legislation requirements, such as PACE (1984).o Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals who contact the service.

Job description

Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support. Support service users to gain access to resources to include benefits, welfare rights, housing and health promotion. Provide compassion to vulnerable individuals at points of acute vulnerability in the criminal justice process. To support criminal justice agencies to better identify vulnerabilities in their respective environments. Provide criminal justice agencies with advice and information regarding pathways to access further support.

To secure referrals into mainstream health and social care services and other relevant interventions and support services from a range of statutory and voluntary providers. Identify levels of risk and escalate to practitioner where appropriate.
