Job summary

We have an exciting opportunity for a Band 4 Nursing Associate / Assistant Practitioner to join our Community Mental Health Team based at Glanville House, Bridgwater. You will play a key role in supporting patients with severe mental illness through our Depot, Clozapine and Wellbeing Clinics, delivering high quality person centred care within a supportive, established and passionate team.

You will work independently under the guidance of Registered Nurses, contributing to holistic mental and physical health assessment, treatment and monitoring. This role is ideal for someone who thrives in a fast paced clinical environment, enjoys collaborative team working, and is passionate about improving the lives of adults with complex mental health needs.

Main duties of the job

  • Support the running of Depot, Clozapine and Wellbeing Clinics, delivering safe and effective clinical care.
  • Undertake venepuncture, physical health monitoring, and ordering medication as required.
  • Work closely with pharmacists, Registered Nurses and wider MDT colleagues.
  • Provide high quality, evidence based nursing care within your scope of practice.
  • Monitor and respond to changes in a patient's mental and physical health, escalating concerns appropriately.
  • Maintain accurate and up to date patient records in line with trust policies and NMC standards.
  • Help maintain a safe, clean and well organised clinic environment.
  • Support patients to improve and maintain their mental, physical and emotional wellbeing.
  • Work collaboratively with colleagues to support safe discharge planning and continuity of care.
  • Contribute to audits, quality improvement and clinical governance activities.

Job description

Clinical Care & Professional PracticeProvide holistic mental and physical health support in our community clinic, including venepuncture, physical observations, medication ordering and preparing patients for procedures. Monitor changes in patient wellbeing, escalate concerns appropriately, and promote health through advice and ongoing support. Act as a patient advocate, ensuring dignity, equality and inclusion.

Teamwork & CommunicationWork collaboratively within the multidisciplinary team, contributing to smooth clinic operations and safe patient care. Communicate clearly with colleagues, patients and families, participate in handovers and MDT meetings, and handle verbal concerns sensitively.

Organisation, Safety & GovernanceManage and prioritise your workload effectively. Adhere to Trust policies on medicines management, infection prevention, safeguarding and confidentiality. Support risk assessments, incident reporting and quality improvement, while ensuring equipment is used and maintained safely.

Learning, Development & SupervisionEngage in supervision, mandatory training and ongoing development. Support students and junior staff where appropriate and work within your professional limits in line with the NMC Code.
