We are looking for a Band 6 Cardiac Rehabilitation Physiotherapist / Exercise Therapist to join out team on a part-time basis.

The role within a multidisciplinary team is to support patients to exercise safely and effectively following a Myocardial Infarction, Coronary Artery Bypass Surgery, Valve repair/replacement and Percutaneous Coronary Interventions in community venues.

To provide highly specialized assessment and exercise prescription for patients with complex needs, and to provide exercise advice, guidance, and information.

To provide a specialized resource for other staff and health care professionals.

To take a lead in the provision of exercise prescription and delivering the exercise component of the Cardiac Rehabilitation Program in a variety of delivery models.

Main duties of the job

  • To assist in the assessment, planning, delivery, and evaluation of effective evidence-based exercise, to patients with coronary artery disease, in community venues.
  • To carry out individual patient functional capacity assessments using standardized exercise assessment tools recognized by British Association for Cardiovascular Prevention and Rehabilitation (BACPR).
  • Work collaboratively within the multidisciplinary team to support patients in achieving their rehabilitation goals and maximising independence.
  • Develop, review and progress rehabilitation programmes in a range of settings ensuring safe and effective practice.
  • Provide specialist advice and education to patients, families, carers and healthcare professionals to support rehabilitation and self-management.
  • To conduct screening and a comprehensive assessment, including interpretation of clinical investigations, conducting appropriate submaximal tests to provide a baseline assess of functional capacity and apply this to exercise programming.
  • To carry out individual mobility assessments and risk stratification.
  • To interpret and evaluate functional capacity modifying individual exercise programs for comorbidities.

Job description

CORE KEY RESPONSIBILITIES

Clinical:

  • Undertakes exercise assessment specific to an individuals requirements. Assimilating complex information requiring analysis and interpretation to select the appropriate exercise assessment tool and exercise prescription.
  • Following patient assessment, liaises with the cardiac nurses, whom, if necessary, can liaise with the patients consultant/GP.
  • Liaises closely with the cardiac nurses to ensure safe effective assessment, planning and delivery of the exercise component of the programme.
  • Prescription of individualised exercise programmes relative to patients physical, psychological, and social needs.
  • Undertake telephone assessment and review and evaluation in support of patients home exercise provision.
  • Responsible for updating all electronic patient records at each contact, ensuring the data recorded is contemporaneous and appropriately communicated to patients GP.
  • Acts as patient advocate and counsel ensuring informed sensitive discussion takes place within the appropriate environment and in the appropriate manner.
  • Meeting and maintaining the highest standards set by the BACPR and contribution to the National Audit of Cardiac Rehabilitation. (NACR)
  • To fully achieve and maintain the clinical competencies required for this post with appropriate ongoing training as required.
  • To co-ordinate and deliver appropriate protocols and procedure relevant to the role in line with national guidelines for Cardiac Rehabilitation programmes.
  • To maintain an accurate and timely record of the treatment provided, the individuals response to the treatment and any modifications of the rehabilitation exercise prescription in accordance with departmental standards.
  • To undertake measurement and evaluation of intervention with goal orientated exercise programmes and outcome measures to progress the individual exercise programme.
  • To ensure exercise programmes are developed in partnership with the patients that respects their choices and values.
