Main duties of the job
Working as part of our multi-professional specialist palliative care team, you will help patients maintain independence, autonomy and quality of life through individualised occupational therapy assessment, intervention and support. You will also play a key role in discharge planning for patients returning home from the Inpatient Unit, working closely with patients, families, carers and colleagues across health, social care, community and voluntary services.
Job description
In this role, you will support patients with activities of daily living, provide advice on symptom management and equipment use, arrange adaptations and equipment where appropriate, and undertake home assessments to support safe and timely discharge. You will also contribute to rehabilitative models of care across hospice settings.
You will act as a specialist resource for colleagues, sharing up-to-date occupational therapy knowledge and promoting evidence-based practice. The role also includes contributing to education, training, service development, clinical governance and the positive promotion of the Hospices values and services.
