Job summary

We are seeking to recruit a Practice Nurse to join our established and highly regarded clinical team on a part-time basis (24-30 hours per week). We are a long-standing, forward-thinking training practice based in Llanelli, known for our supportive culture and strong team ethos.

Main duties of the job

The postholder will be responsible for delivering high-quality nursing care to meet the needs of the practice population. This will include, but is not limited to, wound care, health promotion and preventative care, patient education, and the administration of vaccinations and immunisations.

The role also involves supporting and supervising less experienced clinical staff, contributing to a positive learning environment. The postholder will work collaboratively with clinical, management, and administrative colleagues to support the delivery of safe, effective services and to contribute to the ongoing development and objectives of the practice.

Job description

The postholder will be responsible for delivering a broad range of nursing services to the practice population, including:

  • Providing care for minor injuries, dressings, and complex wound management, liaising with community and secondary care services as appropriate, and escalating to a GP when required.
  • Delivering immunisations in line with national programmes and local guidance, working collaboratively with clinical and administrative colleagues to identify eligible patients and improve uptake, particularly within hard-to-reach groups.
  • Supporting GPs in the management of long-term conditions such as diabetes, COPD, asthma, and hypertension, through the delivery of defined clinical sessions and ongoing patient monitoring.
  • Assisting with seasonal and national vaccination programmes, including flu and COVID-19 campaigns, in line with current guidance.
  • Acting as a chaperone and providing clinical support to colleagues as required.
  • Undertaking and, where appropriate, supervising clinical procedures including anthropometric measurements, blood pressure monitoring, urinalysis, phlebotomy, microbiology sampling, and other practice-based investigations.
  • Delivering health checks in line with national and local initiatives, including establishing and maintaining systems to identify and recall eligible patients.
  • Providing health promotion and patient education, including advice on lifestyle factors such as smoking cessation, physical activity, alcohol use, and diet, and referring to appropriate services where necessary.
  • Promoting patient wellbeing through counselling and education on disease management, medication use, and preventative health measures.
  • Contributing to the development and implementation of clinical audit and quality improvement activities, taking a lead in areas aligned to nursing expertise (e.g. infection prevention and control) and supporting wider practice initiatives.
  • Maintaining up-to-date professional knowledge and skills through ongoing training, development, and participation in relevant educational activities.

Our clinical team comprises 4 GP Partners, 4 Salaried GPs, 2 Clinical Pharmacists, 1 Physician Assistant, 1 Diabetes Specialist Nurse, 2 Practice Nurses, and a Healthcare Support Worker. As an accredited GP Registrar training practice, we are committed to education, development, and maintaining high clinical standards.
