Job summary

The post holder is responsible for the delivery of nursing practice within the homelessness and vulnerably housed communities in Barnstaple and ensuring that everyone is treated with dignity and needs are met in a timely manner. The focus of the role is to bridge the gap of health inequalities and provide education to the communities and professionals of the social determinants within the vulnerable groups.

As an autonomous practitioner the post holder is responsible for the care delivered, demonstrating critical thinking, sensitivity and skills in clinical decision-making in the management of patients.

Working at The Freedom Centre and with other members of the Charity or Council Outreach team to provide Outreach on the streets and in temporary accommodation.

The Inclusion Health Nurse (IHN) is a pivotal member of the primary care team linking with multi-agency teams and supporting emergency and secondary care for patients experiencing homelessness.

The IHN will work with statutory and voluntary services and the acute services to prevent avoidable hospital admissions.

The IHN will support secondary care in facilitating complex discharges.

The IHN will support all learners form other disciplines and agencies by providing shadowing opportunities.

Main duties of the job

The post-holder will:

Location:Based at The Freedom Centre, Barnstaple, with Outreach as required to Supported and Temporary accommodation Settings and people sleeping rough in tents or in cars, caravans or vans, tents, on the streets.
