Job summary

Primary Care Network Care Coordinator

An exciting opportunity has arisen within Primary Care to work as a Care Coordinator in our Churchill, Canbury, Orchard and Berrylands Primary Care Network (CCOB PCN). We care for approximately 50,000 patients in four practices across the borough of Kingston.

You will need a caring and compassionate nature, excellent organisational skills and to enjoy working with people. The world of General Practice is busy and ever changing so an ability to learn quickly and be adaptable to rapid change is essential. The role will require an ability to organise and prioritise your own workload, and so you should be comfortable working independently and as a committed member of a multi-disciplinary team.

Main duties of the job

  • Administration duties- scanning and filing incoming post as well as electronic clinical letters.

Deal with incoming practice email queries directly from hospitals or care agencies.

Manage any admin requests through ANIMA (Triage system) such as medical letters and report requests.

Assist with signposting patients to appropriate community or local services, alongside social prescribers.

Act on communications from hospitals or community providers.

Action and follow death protocol procedure allowing a smooth process for families or close relatives.

Review any new cancer diagnosis patients and consult with our in-house social prescriber for cancer care review.

Manage incoming medical reports and SAR requests (medical record requests) from patients and third parties.

Assist with and ensure patients participate in local or national screening programmes.

Promote health education to our patient population, such as through participation in self-care week.

Pro-actively ensure patients participate in vaccination opportunities such as flu, shingles, or childhood immunisations.

Organise and prioritise own workload.

Monitor tasks to ensure they are completed and care delivered through regular audit of the clinical system.

Directly liaise with multi-agencies to coordinate care for patients.

Support patients and carers and collect feedback on their experiences.

Support the PCN practices to deliver on QOF, KPIs and locally commissioned enhanced services.

To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken.

Participate in practice meetings, patient participation groups and other meetings to improve engagement with the role.
