St Catherines Hospice is a respected local charity providing specialist hospice care alongside statutory service partners (GPs, District nursing etc) across West Sussex and East Surrey.

This specialist role provides expert support to patients and families in the Community.

Main duties of the job

Are you an experienced clinician with a passion for delivering specialist palliative care? Are you an effective communicator, with substantial post-registration experience, with expertise in Palliative Care?

Do you bring confidence, resilience and a compassionate approach?

We are seeking a passionate leader with sound knowledge of end of life care with confidence in care planning and individual assessments. If you have computer literacy and experience teaching and developing others we would love to hear from you!

Clinical Nurse Specialist will bring:

  • Ability to listen, plan and communicate
  • Strong quality and governance awareness
  • Working as part of an integrated team of clinicians
  • Undertaking specialist palliative care assessments, and specialising in the advisory and Care Coordinator component of this support

This post is offered on a part time, permanent basis, working 28.5 hours per week. Hours worked three shifts per week, over seven days on a rota basis, working 9.5 hours each day. We operate a 365 day service and offer pay enhancements for unsociable hours and weekend working.

Please note, the successful candidate will be an essential car user, therefore candidates must have a full driving licence and access to a vehicle.

Job description

Key Responsibilities:

Support and take part in the St Catherines telephone advice line

Manage and co-ordinate all aspects of the patient pathway through our services to include ensuring that we continue to provide high standards of holistic and individualised specialist patient care embracing the hospice's vision and values

Working with external clinical colleagues ensure that every patient receives a comprehensive nursing assessment and has individualised care plans to reflect their patient centred care needs

To be responsible for the continued co-ordination of the content and delivery of your allocated patients care plans, and assure that patients receive the right service from the right team both internally and externally

In liaison with other clinicians ensure the overall quality and provision of nursing care of patients and families referred to St Catherines in the community setting have their needs met

To promote the principles and practice of specialist palliative care as an extension of the hospice into the community working in collaboration with local neighbourhoods

To work across a range of settings, including community visits to patients homes care homes community hospitals and outpatient services

To assess the needs of patients and their carers and identify their respective physical psychological social cultural and spiritual needs

Be responsible for assurance of standards and clinical governance standard setting and audit

To assess peoples information needs and provide relevant information to meet those needs

To work within a team framework collaborating with colleagues and members of other disciplines

To use a proactive and sensitive approach with patients and relatives to enable involvement in advance care planning

To offer bereavement support to carers in collaboration with the Wellbeing team referring them onto appropriate services as required

To provide appropriate nursing interventions and care

To contribute to patient management governance and liaison by attending Gold Standard Framework (GSF) hospital and other practice and community based meetings

To liaise and communicate with community nursing home and hospital staff in order to raise awareness and promote hospice services

To foster positive working relations and work alongside Primary Healthcare Teams and with other health care professionals in the delivery of community based palliative care
