A minimum of 12 months remaining on your right to work in the UK and at least 1 year administrative experience are required. Experience in care would be desirable but not necessary. The role includes coordinating care services, matching carers with clients, planning rotas, recording staff availability, and participating in on-call rotations.

The role of a Care Coordinator

Coordinate care services and client schedules
Match carers with clients to meet individual needs
Support the setup of new care packages
Plan rotas to ensure continuity of care
Communicate with families and healthcare professionals
Record staff availability, absences, and emergency calls
Participate in the on-call rota when required
Willingness to cover the on-call phone (additional pay provided)
Role is predominantly office based, with some need to deliver care within an extra care setting
