Job summary

The role is responsible for managing and coordinating the pathway of patients within Urology cancer and benign planned care pathways at The Queen Elizabeth Hospital King's Lynn NHS Foundation Trust. Initially focusing on delivery and support of the PSA Remote Surveillance Monitoring, the role will extend to all urology cancer and complex benign pathways.

The post holder will proactively track and expediate urological patients from referral through to diagnosis, treatment and follow-up, ensuring timely progression in line with national cancer targets and local service requirements. This includes booking appointments and diagnostics, liaising with clinicians, MDT coordinators (MDTC), Patient Pathway Coordinators (PPC) and partner services to ensure patients are diagnosed within the timeframes outlined in national guidance.

Main duties of the job

Throughout the person living with cancer's journey from diagnosis, to living with and beyond, the Urology Cancer Navigator will direct the person and family to relevant information resources and to other organisations information resources to meet their information needs.

Support people to access local appropriate information and sign-posting to a range of support services, an approach which helps people to self- manage where appropriate.

Following cancer diagnosis, ensure every patient detail is added to the project database, to enable the support of all patients from the start of their cancer journey. Record all patient contacts with Somerset Cancer Registry.
